Request a Transcript or Credential

Bishop Kelly partners with Parchment to order and send your transcript and other credentials, such as Verification of Compliance documents, securely.

Verification of Compliance Documents, which are necessary for students to obtain their driver’s permit, will be sent to your email upon ordering. You can immediately print the VOC and take it with you to the DMV.

To request a St. Teresa’s Academy transcript:

Transcripts from St. Teresa’s academy must be requested directly from the Sisters of the Holy Cross.

Download and print out THIS FORM on the Sisters of the Holy Cross website.

Return the form with $3 to:

Transcripts
100 Rosary Hall
Notre Dame, IN 46556-5050

Form must be printed out and mailed with a $3.00 payment.  Make checks payable to Sisters of the Holy Cross.
